Mentally disabled woman Barbara Dziubczynski was not adequately supervised and fell down numerous times, according to a lawsuit filed in Will County court.

A mentally disabled woman in the care of Cornerstone Services fell 17 times in three months before dying of her injuries, according to a lawsuit filed in Will County court.

The lawsuit filed on behalf of Barbara Dziubczynski accuses Cornerstone and two nurses—Sheila Norwood and Lisa Bravo—of failing to adequately supervise her.

Dziubczynski, 61, lived at a Cornerstone house on Coral Bell Drive in Joliet, the lawsuit said. Between July and October 2011, Dziubczynski fell down at least 17 times while in the house, the suit said.

Dziubczynski's falls led to a "prolonged hospitalization," the suit said, and on Oct. 22, 2011, she suffered a subdural hematoma during a fell. This injury ultimately killed her, according to the lawsuit.
