Free Sports Physicals to Current Minooka High Students

Per Illinois High School Association (IHSA) regulations, all high school student-athletes must have a current physical on record with the school’s principal in order to participate in school athletics during the 2011-2012 school year.

In an effort to help student-athletes meet this IHSA regulation, MCHS, in cooperation with Dr. Rezin Orthopedic and Dr. Benig of Shorewood Medical Practice, will offer free sports physicals on a first-come first-serve basis for all MCHS students on Saturday, February 25, from 8 a.m. to 11 a.m. at MCHS –South Campus (enter through Door Twenty).  Prior to the physical, student-athletes must acquire an IHSA physical form either from the MCHS web site (visit www.mchs.net, click on “Forms” in the “Quick Links” menu, and then select the Athletic Participation Physical form in the Athletics section to download and print) or the IHSA web site (www.ihsa.org).  These forms must be fully completed and signed by a parent or guardian and the student-athlete before the physical will be performed.  These physicals will meet the IHSA requirement for one calendar year. After the physical is completed, the form will be filed that day with the school nurse.

If you have any questions, please call the athletic office at 815-467-2140 ext. 135.
